The standard complaint form used by the CSLB may be obtained by writing or calling the nearest district office. CSLB maintains 15 such offices throughout the state. District offices are listed in the white pages of local telephone directories under "State of California".
When you have filled out the complaint form, attach copies of contracts, canceled checks, or other information pertinent to your complaint and return it to the district office.
